RESUMO

The popular immediate dentin sealing (IDS) technique is used to improve the bond strength of indirect restorations. This systematic review assessed whether bond strength is affected by the type of aging conditions, bonding agents, flowable resin composites, impression materials, temporary materials, and/or resin cement used within the IDS procedure. A comprehensive database search of PubMed, Embase, Scopus, Ovid Medline, Web of Sciences, Cochrane Library, Dentistry & Oral Sciences Source, and ProQuest was carried out up to 30 January 2024 without publication year or language limitations. Only in vitro full-texts regarding the effect of IDS on bond strength were included, and the quality of their methods was assessed via a Risk of Bias (RoB) test. In total, 1023 pertinent studies were initially found, and 60 articles were selected for review after screening for the title, abstract, and full texts. IDS application improves the bond strength of indirect restorations to dentin and reduces the negative effects of temporary materials on the bond durability of final indirect restorations. Filled dentin bonding agents or combinations with flowable resin composite are preferred to protect the IDS layer from conditioning procedures.

OBJECTIVES: Aim of this prospective study was to assess full mouth rehabilitation of severe tooth wear patients using minimally invasive CAD/CAM resin-based composite (RBC) restorations and direct veneers by evaluating restoration survival up to 5.5-years. METHODS: Twenty-two patients with generalized severe tooth wear with functional and/or esthetic problems were included. Following minimally invasive preparation, CAD/CAM RBC restorations (LAVA Ultimate,3M) were adhesively luted, direct RBC veneers (Filtek Supreme XTE, 3M) were applied in the aesthetic region. Patients were recalled after 1m,1y,3y,5y and seen in between recalls by their general dentists or at the clinical study center if complaints occurred. Failures were categorized as F1 (severe deficiencies requiring replacement/extraction), F2 (localized deficiencies requiring re-cementation/repair) and F3 (small chippings requiring refurbishment/monitoring). Survival of indirect restorations was evaluated using lifetables and Kaplan-Meier-graphs, distinguishing between failure categories and tooth type (front teeth=FT, premolars=PM, molars=M). F1 + F2 and F1 + F2 + F3 failures were analyzed using Cox regression on the variables tooth type/ location, age, gender and VDO increase (p < 0.05). RESULTS: 568 indirect restorations and 200 direct veneers in 21 patients evaluated for up to 5.5-years. For indirect restorations, 96 failures were recorded (F1:6;F2:41;F3:49) and annual failure rates were 0.29%(FT), 1.56%(PM), 2.93%(M) for F1 +F2 and 0.53%(FT), 2.42%(PM), 6.11%(M) for F1 + F2 + F3. Reasons for failure were chipping fracture (48), adhesive fracture (32), complete debonding (7), caries (4), endodontic treatment (1) and reasons unknown (documentation general dentists, 4). Molar tooth type had a statistically significantly increased probability of failure compared with front teeth and premolars for F1 + F2 + F3 (p < 0.006). Direct veneer restorations showed 18 failures (F1:2;F2:9;F3:7). SIGNIFICANCE: Minimally invasive CAD/CAM RBC restorations combined with direct RBC veneers showed an acceptable clinical mid-term survival for restorative rehabilitation of severely worn dentitions.

Determining the appropriate technique for diastema closure is challenging, and the decision must be evidence based. The objective of these case reports is to describe different approaches to diastema closure using direct and indirect techniques, focusing on the characteristics of the patient and clinical requirements to guide treatment. In the first case, a 16-year-old patient had multiple diastemas in the maxillary anterior dentition. The clinical evaluation revealed microdontia of the lateral incisors and malpositioned teeth. The treatment included tooth whitening and placement of composite resin veneers using a direct technique. In the second case, a 54-year-old patient displayed a disharmonious and esthetically compromised smile due to small teeth, color changes, multiple diastemas, incisal wear, and severe dentogingival disproportion. Based on the patient's expectations, the patient's age, and the presence of a "black triangle" interdental space, a multidisciplinary restorative treatment was proposed, including gingivoplasty, tooth whitening, and placement of ceramic laminate veneers using an indirect technique. Both approaches achieved successful esthetic rehabilitation and diastema closure with minimal intervention. The choice of procedure and restorative material, as well as the need for tooth preparation, varied based on the clinical requirements, patient expectations, and financial constraints. Careful treatment planning avoided lengthy and inefficient procedures.

INTRODUCTION: The purpose of this systematic review and meta-analysis was to evaluate and compare the failure rates of direct and indirect restorations for single-tooth restorations. METHODS: A literature search was conducted by using electronic databases and relevant references for clinical studies on direct and indirect dental restorations with a follow-up of at least 3 years. The risk of bias was assessed with the ROB2 and the ROBINS- I tools. The I2 statistic was used for the assessment of heterogeneity. The authors reported summary estimates of annual failure rates of single-tooth restorations using a random-effects model. RESULTS: Of 1415 screened articles, 52 (18 RCTs, 30 prospective, 4 retrospective) met the inclusion criteria. No articles with direct comparisons were identified. No significant difference was found in the annual failure rates of single teeth restored with either direct or indirect restorations, which were calculated as 1% using a random-effects model. High heterogeneity was found, ranging from 80% (P⟨0.01) for studies on direct restorations to 91% (P⟨0.01) for studies on indirect restorations. Most of the studies presented some risk of bias. CONCLUSIONS: Annual failure rates were similar for direct and indirect single-tooth restorations. Further randomized clinical trials are needed to draw more definitive conclusions.

O objetivo deste estudo foi analisar a resistência de união de restaurações indiretas cimentadas em superfícies dentinárias desmineralizadas com simulação da remoção seletiva da dentina com e sem uso de material forrador. Foram utilizados 60 dentes bovinos com a superfície vestibular desgastada até dentina sadia e divididos em quatro grupos: dentina sadia (controle), dentina desmineralizada (des), dentina desmineralizada e selada com resina flow (des-flow) e dentina desmineralizada e coberta com cimento de inonômero de vidro (des-civ). Todos os espécimes receberam condicionamento com ácido fosfórico (37% Condac, FGM) previamente a hibridização com adesivo universal (Single Bond Universal; 3M). O grupo des-civ recebeu uma cobertura de cimento de ionômero de vidro previamente ao condicionamento e hibridização e o grupo des-flow recebeu um selamento com resina flow (Resina Filtek Z350 XT Supreme Flowable, 3M) após condicionamento e hibridização. Em sequência, foram cimentadas peças simulando restaurações indiretas em resina composta (Grandioso ­ VOCO) na região condicionada de cada amostra. O conjunto recebeu fotoativação por 60 segundos em cada face da peça cimentada. Após 48 horas da cimentação, os espécimes dos três primeiros grupos foram submetidos aos testes mecânicos de tração, com velocidade de 1 mm/min. E após 29 dias, o quarto grupo foi submetido ao mesmo teste. A análise do tipo de fratura foi realizada por estereomicroscopia e os dados foram convertidos em MPa. Os resultados apresentaram diferença estatística de acordo com o teste ANOVA a 1 fator (p=0,011). Para as comparações múltiplas, o teste de Tukey apontou que o grupo des-flow apresentou valores menores do que o grupo controle. Todas as outras comparações foram similares. Concluiu-se que o grupo des-flow, quando comparado ao grupo controle, apresentou uma adesão inferior e que os materiais forradores não diferiram entre si (AU)


The aim of this study was to analyze the bond strength of cemented indirect restorations on demineralized dentin surfaces by simulating selective dentin removal with and without the use of a liner material. Sixty bovine teeth were used with the buccal surface worn down to healthy dentin and divided into four groups: healthy dentin (control), demineralized dentin (des), demineralized dentin sealed with flow resin (desflow) and demineralized dentin covered with glass ionomer cement (des-civ). All specimens were etched with phosphoric acid (37% Condac, FGM) prior to hybridization with universal adhesive (Single Bond Universal; 3M). The des-civ group received a glass ionomer cement cover prior to etching and hybridization, and the desflow group received a flowable resin seal (Filtek Z350 XT Supreme Flowable Resin, 3M) after etching and hybridization. Subsequently, specimens simulating indirect composite resin restorations (Grandioso - VOCO) were cemented in the etched region of each sample. The set was photoactivated for 60 seconds on each side. 48 hours after cementation, the specimens from the first three groups were subjected to mechanical tensile testing at a speed of 1 mm/min. After 29 days, the fourth group was subjected to the same test. The analysis of the fracture type was performed by stereomicroscopy and the data were converted into MPa. The results showed statistical difference according to the one-way ANOVA (p=0.011). For multiple comparisons, the Tukey's test showed that the des-flow group presented lower values than the control group. All other comparisons were similar. It was concluded that the des-flow group, when compared to the control group, presented lower adhesion and that the liner materials did not differ among themselves.(AU)

The purpose of this review was to analyze the existing literature on surface conditioning of the veneering surface of substructure restorative materials in dental laboratories. New technologies are constantly improving the treatment options for fabricating dental restorations, and new materials and adhesion procedures are being offered to clinicians and dental technologists. To establish a reliable adhesion between the veneer and substructure in the dental laboratory, various surface treatment procedures and adhesion promoters are employed. The composition of a material influences the adhesion approach selected, and implementing a reliable adhesion strategy is critical for the predictability of veneered indirect dental restorations. However, surface treatment of a wide range of available material options can be challenging. Therefore, understanding various adhesion processes for different restorative materials may assist dental technologists in selecting the best and appropriate surface conditioning protocol for each dental restorative material category.

BACKGROUND: Evaluating the effect of different surface treatment methods on the micro-tensile bond strength (µTBS) of two different resin-matrix computer-aided design/computer-aided manufacturing (CAD/CAM) ceramics (RMCs). METHODS: A standardized inlay preparations were performed on 100 intact maxillary premolars. According to the type of the restorative material, the teeth were randomly divided into two equally sized groups (n = 50): (polymer-infiltrated ceramic (Vita Enamic) and resin-based composites (Lava Ultimate)). The inlays were fabricated using CAD/CAM technology. In each group, the specimens were randomly assigned to five subgroups (n = 10) according to the surface treatment method: group 1 used was the control group (no surface treatment); group 2, was treated with air abrasion with 50 µm Al2O3 (A) and universal adhesive (UA); group 3, was treated with air abrasion with 50 µm Al2O3 (A) and silane coupling agent (S); group 4, was treated with hydrofluoric acid (HF) and universal adhesive (UA) and group 5, was treated with Hydrofluoric acid (HF) + silane coupling agent (S). The inlays were then cemented to their respective preparations using dual-cure self-adhesive resin cement (RelyX U200, 3 M ESPE) according to the manufacturer's instructions. The µTBS test was conducted in all groups, and stereomicroscope and scanning electron microscope were used to inspect the failure mode. The data were statistically analyzed using a two-way analysis of variance (ANOVA) and Tukey's post-hoc multiple comparison tests at a significance level of p < 0.05. RESULTS: Surface treatments significantly increased the µTBS of the materials compared to the control group (p < 0.05). For CAD/CAM RBCs, the µTBS value highest in group 2 whereas, for PICN, the µTBS value was highest in group 3. Cohesive failure of CAD/CAM restorative material was the most predominant mode of failure in all treated groups, whereas adhesive failure at restoration-cement interface was the most predominant failure mode in the control group. CONCLUSION: Surface treatments increase the µTBS of resin-matrix CAD/CAM ceramics to tooth structure. Air abrasion followed by universal adhesive and hydrofluoric acid followed by silane application appears to be the best strategies for optimizing the bond strength of CAD/CAM RBCs and PICN respectively.

The aim of the present study was to evaluate the fatigue to cyclic and static resistance of indirect restorations with different preparation designs made either of lithium disilicate (LS) or polymer-infiltrated ceramic network (PICN). Eighty-four (n = 84) molars were chosen, endodontically treated, and prepared with standardized MOD cavities. The molars were randomly divided into 6 study groups (n = 14) taking into account the "preparation design'' (occlusal veneer with 1.2 mm occlusal thickness; overlay with 1.6 mm occlusal thickness; adhesive crown with 2 mm occlusal thickness) and the "CAD/CAM material'' (E-max CAD, Ivoclar vivadent; Vita Enamic, Vita). A fatigue test was conducted with a chewing simulator set at 50 N for 1,500,000 cycles. Fracture resistance was assessed using a universal testing machine with a 6 mm diameter steel sphere applied to the specimens at a constant speed of 1 mm/min. A SEM analysis before the fracture test was performed to visually analyze the tooth-restoration margins. A statistical analysis was performed with a two-way ANOVA and a post-hoc pairwise comparison was performed using the Tukey test. The two-way ANOVA test showed that both the preparation design factor (p = 0.0429) and the CAD/CAM material factor (p = 0.0002) had a significant influence on the fracture resistance of the adhesive indirect restorations. The interaction between the two variables did not show any significance (p = 0.8218). The occlusal veneer had a lower fracture resistance than the adhesive crown (p = 0.042) but not lower than the overlay preparation (p = 0.095). LS was more resistant than PICN (p = 0.002). In conclusion, in the case of endodontically treated teeth, overlay preparation seems to be a valid alternative to the traditional full crown preparation, while occlusal veneers should be avoided in restoring non-vital molars with a high loss of residual tooth structure. LS material is more resistant compared to PICN.

RESUMO

Zika virus congenital syndrome (ZVCS) is a congenital viral infection resulting from the transmission of the Zika virus (ZV) to the fetus during pregnancy. This report describes a clinical case involving a 20-month-old female child with ZVCS, who presented with systemic changes related to the syndrome, such as microcephaly, arthrogryposis, ocular and auditory changes, and oral changes such as delayed dental eruption, ogival (high-arched) palate, short lip frenum, and altered morphology of a superior primary incisor. For esthetic and functional rehabilitation of the oral health of this child, an indirect composite resin restoration was performed using intraoral digital scanning technology. This case presents an accurate, rapid, and comfortable restorative treatment option that might result in excellent outcomes in children with ZVCS or similar syndromes with neurological impairment.

OBJECTIVE: In this retrospective clinical study, we investigated the long-term survival and success of indirect gold restorations, placed by undergraduate students. MATERIAL AND METHODS: The study included 72 gold restorations in 45 patients, placed in a five-year period between 2009 and 2013. Two experienced dentists performed the evaluation following the FDI criteria after 5 to 10 years. Kaplan-Meier test was used for longevity analysis of success and survival. Data were tested for normality with Shapiro-Wilk test. Cox regression analysis, with significance level appointed at p ≤ .05, was performed using success and survival of restorations as dependent variables. Annual failure rates (AFRs) were calculated. RESULTS: Overall, 72 indirect gold restorations prepared in posterior teeth were assessed, with an observation time from 63 months to 117 months. The mean service time was 88.1 months and the median 87 months. No failures, but 2 repairs, were observed during the first five years. From 5 to 10 years 1 replacement and 1 more repair occurred. The survival and success rates of gold restorations were calculated at 9 years with 98.6% and 91%, respectively. Restorations placed on premolars had higher survival and success rates compared to molars. Three cases were categorized as repairs: in two cases, a root canal treatment was needed and, in one other, an occlusal perforation of the restoration was observed. CONCLUSIONS: Posterior indirect gold restorations placed by dental undergraduate students showed a highly predictable clinical performance in terms of success and survival. These results are comparable with survival rates reported in literature for restorations cemented in general dental practices. CLINICAL SIGNIFICANCE: Casted gold restorations placed from dental students in an undergraduate setting show high survival rates, similar to professional dentists.

Objective: The restoration durability is essentially governed by optimum marginal integrity of an indirect restoration that is obtained and maintained by essential factors. This study aimed to evaluate the internal gap of indirect restorations fabricated from CAD/CAM composite blocks versus ceramic blocks in badly broken teeth using cone beam CT (CBCT) to determine their internal fit accuracy over the cast. Materials and methods: Fifty-four participants were allocated into two groups: composite blocks or ceramic blocks. The trial participants and assessors were blinded to the material assignment, whereas the operator was not. Cavity preparation was performed followed by cavity optimization and impression taking. The produced master cast was scanned, restoration was designed using Exocad 2019 software and the final restoration was milled. The restoration was doubled-checked on the cast for internal fit using cone beam computed tomography (CBCT), and intraorally for interproximal contact and marginal adaptation before final cementation. CBCT measurements were collected and statistically analyzed. Restoration was cemented with resin cement and was immediately assessed clinically, then after one year and two years of follow-up periods. Results: Results of an independent t-test revealed Composite blocks samples (0.25mm ± 0.03) to have a significantly better adaptation than ceramic blocks samples (0.29 mm ± 0.04) (p = 0.008). Conclusions: Both materials have acceptable internal adaptation with a noticed difference reflected in their clinical performance. Clinical relevance: Both indirect esthetic CAD/CAM restorations exhibit acceptable internal and marginal adaptation in posterior teeth however, composite blocks have a better adaptation than ceramic blocks. OBJECTIVE: To evaluate clinical performance of the new CAD/CAM resin-matrix ceramics and compare it with ceramic partial coverage restorations. MATERIALS AND METHODS: An electronic search of 3 databases (The National Library of Medicine (MEDLINE/PubMed), Scopus, and the Cochrane Central Register of Controlled Trials) was conducted. English clinical studies published between 2005 and September 2020 that evaluated the clinical performance of CAD/CAM resin-matrix ceramics inlays, onlays, or overlays were selected. The primary clinical question was applied according to PICOS strategy (Population, Intervention, Comparison, Outcome, Study design). The included studies were individually evaluated for risk of bias according to the modified Cochrane Collaboration tool criteria. RESULTS: A total of 7 studies were included according to the established inclusion and exclusion criteria. From the included studies, 6 were randomized clinical trials while one study was longitudinal observational study without control group. According to the results of the included studies, the success rate of CAD/CAM resin-based composite ranged from 85.7 to 100% whereas the success rate reported for ceramic partial coverage restorations ranged from 93.3 to 100%. Fractures and debondings are found to be the most common cause of restorations failure. CONCLUSION: CAD/CAM resin-based composite can be considered a reliable material for partial coverage restorations with clinical performance similar to glass ceramic restorations. However, this result needs to be confirmed in long-term evaluations. CLINICAL RELEVANCE: CAD/CAM resin-based composites provide a potential alternative to ceramic indirect restorations. However, clinicians must be aware of the lake of knowledge regarding long-term outcome.

OBJECTIVES: This systematic review investigated the hypothesis that preheated resin composites (RCs) used as luting agents improve the mechanical properties, physicochemical performance, and color of indirect ceramic restorations. MATERIALS AND METHODS: Literature search was performed in three databases (Medline/PubMed, Scopus, and Web of Science) and in the grey literature (OpenGrey, ProQuest, and Catalog of Theses & Dissertations from CAPES). Eligibility criteria included only studies comparing at least one preheated RC used as a luting agent with resin cements. RESULTS: Data regarding the mechanical properties, physicochemical characteristics, and color were analyzed qualitatively, and the microtensile bond strength and film thickness were also evaluated by meta-analysis. The search strategy identified 3894 papers, and 28 were full-text screened. Seven studies were included in the review, and 5 were included in the meta-analysis. No significant difference was found for microtensile bond strength (P = 0.14). Preheated RCs showed significantly higher film thickness than resin cements (P = 0.001). CONCLUSION: Overall, the use of preheated RCs as luting agents offers similar to poorer performance than using resin cements for bonding indirect restorations. CLINICAL RELEVANCE: Despite the claim that preheated RC could be used as a luting agent, further studies should investigate the effect of clinically unacceptable film thickness.

AIM: The aim of this systematic review is to compare the bonding performance of indirect restoration with the reinforced immediate dentin sealing (IDS) method as opposed to the conventional IDS method. MATERIALS AND METHODS: A literature search was conducted in PubMed, Cochrane, and EBSCOHost up to January 31st 2022, accompanied by a hand search in Google Scholar. Inclusion criteria involved studies comparing conventional IDS and reinforced IDS protocol and evaluating various parameters influencing the bonding performance, such as type of indirect restoration, etching protocol, cavity design, tooth surface preparation, method of oral cavity simulation, and processing after luting. The quality of six included studies was appraised using CRIS guidelines. RESULTS: A total of 29 publications was identified, and 6 of them fulfilled the inclusion criteria. All of the included studies were in vitro studies. The predetermined data were independently extracted and evaluated by four reviewers. It was observed that most of the studies showed an improvement in bond strength with reinforced IDS when compared with conventional IDS. Also, etch-and-rinse and 2-step self-etch adhesive protocols have shown better bonding performance than universal adhesive systems. CONCLUSION: Reinforced IDS has similar or better bond strength to that of conventional IDS strategies. The need for prospective studies is highlighted. The future clinical studies for immediate dentin sealing ought to be reported in a uniform and methodological way. CLINICAL SIGNIFICANCE: Application of an additional layer of low-viscosity resin composite provides a thicker adhesive layer, prevents re-exposure of dentin during the final restoration, and allows a smoother preparation in lesser clinical chair time and eliminates any possible undercuts. Thus, reinforced IDS has shown to result in better preservation of the dentinal seal than IDS technique.

BACKGROUND: The interest in vertical crown preparation is growing and several variations are presented in the dental literature but limited clinical outcome data exists for teeth prepared for a vertical finish line. AIM: To audit clinical outcomes for 73 teeth after vertical preparation for knife-edge zirconia crowns. METHOD: Seventy-three teeth had knife-edge zirconia crowns placed after vertical finish line preparation. The outcomes for these teeth were analysed from a retrospective audit of clinical records. RESULTS: The mean follow-up time was 21 months (SD: 10 months; range: 6-40). One tooth had to be extracted due to fracture at cervical level. One crown had to be re-made after margin fracture during try-in. No subsequent endodontic treatment was needed for any of the prepared teeth. No change in alveolar bone height was noted on radiographic follow-up. The mean bleeding score for the crowned teeth was higher than the mean bleeding score for a control tooth, but this was not statistically significant. CONCLUSION: This retrospective evaluation has shown favourable outcomes for 72 teeth after vertical preparation for knife-edge crowns. Longer follow-up is needed but the present results show that the technique is a viable procedure with potential advantages.

PURPOSE: The immediate application of a dentin-bonding agent after tooth preparation and before impression-taking (immediate dentin sealing [IDS]) has been suggested to provide several advantages concerning bacterial microleakage, hypersensitivity, and bonding quality. We reviewed the literature and clarified certain aspects related to each step of IDS application. MATERIALS AND METHODS: The search strategy comprised an electronic research in MEDLINE, Cochrane, Ovid and Scopus for studies published from January 1990 to December 2020 regarding the IDS technique and including both in vitro and clinical studies. RESULTS: After exclusion of irrelevant or duplicate articles, 88 articles focusing on aspects of the IDS technique were assessed. IDS seems to be advantageous with regard to bond strength, gap formation, bacterial microleakage, and dentin hypersensitivity. However, issues arising from interaction with impression materials, the provisional phase, and conditioning methods before cementation require further investigation. CONCLUSION: There are no documented reasons preventing clinicians applying IDS in their everyday practice. On the contrary, the presented technique seems to be beneficial in certain aspects regarding indirect restorations.

Vários fatores podem influenciar o desempenho clínico de dentes com tratamento endodôntico (DTE). Além disso diversas são as opções restauradoras indicadas em respeito a este cenário, em especial com o notório avanço de novas técnicas e tecnologias para uso clínico. Dentre estas corriqueiramente envolvem-se o uso de restaurações diretas (confeccionadas com resina composta), pinos metálicos ou de fibra de vidro, pinos pré-fabricados ou fresados, assim como restaurações indiretas compostas de coroas cerâmicas, inlays, onlays ou endocrowns; sempre com o foco na reabilitação funcional e estética do caso em tela. Neste trabalho, através de uma revisão narrativa, foram expostos conceitos relativos às modalidades restauradores que se advêm do uso de novas tecnologias na reabilitação de dentes tratados endodonticamente(AU)


Several factors may influence the clinical performance of endodontically treated teeth (ETT). In addition, there are several restorative options indicated regarding this scenario, especially with the notable advance of new techniques and technologies for clinical use. These routinely involve the use of direct restorations (made of composite resin), metal or fiberglass posts, prefabricated or milled posts, as also indirect restorations composed of ceramic crowns, inlays, onlays or endocrown; always focusing on functional and aesthetic rehabilitation of the case in hand. In this study, through a narrative review, concepts related to the restorative modalities that were introduced with the use of new technologies in the rehabilitation of endodontically treated teeth were discussed(AU)

PURPOSE: This study was done to compare the survival rates of cast gold and ceramic onlays placed in a dental school setting. MATERIAL AND METHODS: An electronic search was conducted in the patient records at Adams School of Dentistry, University of North Carolina at Chapel Hill for onlay codes that were in the database (From 1998 until 2018). Progress notes and radiographs were scrutinized to establish the survival time of the restorations. Any complications that occurred during the life time of the restorations were noted. The survival was summarized by categorization based on ranges of survival time in years; group 1: 1 to 5 years, group 2: 6 to 22 years. The mean survival time and standard deviation were calculated. One-way ANOVA was used to determine whether there was a statistically significant difference in the survival times between gold and ceramic onlays. RESULTS: The mean survival rate of cast gold onlays (86.6%) was comparable to that of ceramic onlays (81.1%). The gold onlays in Group 1 had a higher mean survival time (2.43 years) than the ceramic onlays (2.03 years). This difference was statistically significant (p = 0.002). The ceramic onlays in Group 2 had a mean survival time of 19.75 years while gold onlays had a mean survival time of 17.63 years. This difference was not statistically significant (p = 0.91). CONCLUSION: It was concluded that while the survival rate of ceramic onlays (81.1%) was inferior to that of cast gold onlays (86.6%), it was comparable.

BACKGROUND: Short-fiber-reinforced resin composite (SFRC) is a direct bulk fill resin composite specially indicated in large complex cavities. It is characterized by having high fracture toughness and load bearing capacity to decrease the incidence of fracture of the restoration. MATERIALS AND METHODS: In two parallel groups (n = 38 restorations), 76 participants having complex proximal cavities with asymptomatic vital pulp were randomly enrolled in this trial and received either SFRC (Ever X Posterior, GC, Japan) covered by Gaenial posterior (GC, Japan) or chairside indirect restorations (Grandioso inlay system; VOCO, Germany) fabricated on a silicon die. Materials were applied according to the manufacturer instructions with the corresponding adhesive system. Only 67 participants completed the trial, which was assessed using the modified United States Public Health Service (USPHS) criteria by two independent blinded assessors at 6 months and 1 year follow-up visits. STATISTICAL ANALYSIS: Fisher's exact and Cochran's Q tests were used to analyze inter- and intragroup comparisons, respectively. The significance level was set at p ≤ 0.05. RESULTS: No statistically significant difference was observed between both tested groups for all USPHS criteria at different follow-up periods except for marginal integrity favoring the SFRC at 12 months when the difference became significant (p < 0.001), and color match favoring the nanohybrid indirect resin composite restorations with significant difference in scores at all follow-up intervals (p < 0.001) was found. Cochran's Q test showed significant differences within the same technique during the follow-up period for some criteria. CONCLUSION: Direct SFRC and indirect nanohybrid resin composite complex proximal restorations showed an acceptable clinical performance along the 1 year follow-up period. CLINICAL RELEVANCE: Direct SFRC restorations could be a viable treatment option for complex restorative cases.

Dental caries is the major oral health problem in most of the countries, affecting 60-90% of school children and a vast majority of adults. Therefore, it is of interest to evaluate the association of age with Class VI defects restored with composite restorations. We used 102 cases with data regarding Class VI composite restorations in a datasheet of 86,000 records at Saveetha Dental College, India for this study. Data shows that Class VI restorations were commonly seen in upper anterior teeth in the age group of 51 and above. The cavities prepared to receive Class VI restoration followed a conservative design of caries removal and used direct restoration techniques for reconstruction of the lost tooth structure.
